Abstract: 本研究は,小児中核病院の看護師および医療ソーシャルワーカーが,医療的ケア児の在宅移行支援において捉える課題と部署間での差異を明らかにすることを目的とした.医療的ケア児にかかわる看護師22名と医療ソーシャルワーカー2名を対象に,半構造化によるフォーカス・グループ・ディスカッションを行った.その結果【子どもと家族にかかわる時間を捻出できない】,【有機的に看護がつながるシステムになっていない】,【移行時に家族が混乱をきたす/きたしている】,【在宅移行後の様子について把握できない/できていない】,【退院調整に時間がかかっている】,【子どもおよび家族を支える資源につなげられない】,【在宅移行を見通したチームアプローチができていない】と,7つのカテゴリが抽出された.医療機関完結ではなく,在宅まで含む多職種連携チームアプローチの意識を持ち,在宅移行支援体制整備を推進し,共通理解を深める必要が示唆された.
This study aims to clarify the issues perceived by nurses and medical social workers who support children with medical complexity and their families in transition to home care from the core pediatric hospital. It does so by focusing on differences between departments. Semi-structured focus group discussions were conducted with 22 nurses and 2 medical social workers who take care of children with medical complexity. Seven categories were extracted: “unable to find time to be involved with children and families”; “not an organic nursing connection”; “family confusion at the time of transition”; “unable to ascertain how children and families live after transition to home care”; “discharge coordination takes time”; “unable to connect patients to resources to support these children and their families”; and “lack of a forwardlooking interdisciplinary team approach.” Our findings suggest that rather than addressing everything within the medical institution, it is important to deploy a multidisciplinary team-based approach that includes the child’s home life, thereby developing a home transition support system and deepening mutual understanding.
